Learning objectives: Penetrance for gastrointestinal hereditary cancer syndromes varies tremendously. For the surgeon it means indvidual risk evaluation depending on the underlying syndrom and could mean early prophylactic proctocolectomy in the teens or even earlier or yearly screening. In all decisions it is about optimizing quality of life and prevention of avoidable cancers following the principle of 'as little as possible but definitely as much as necessary'.
